The program is available to independent agents. In an instance where the same submission is received from more than one broker, a Broker of Record letter determines control.

TO OBTAIN A QUOTATION

Simply mail a submission to WellGuard. Lead time needed: 60 days. The submission must be complete and meet the underwriting guidelines and eligibility criteria. The submission should include the following:

• Completed Acord applications, signed by the insured, including detailed property information to include construction, occupancy, square footage and age
• WellGuard Supplemental Application, signed by the insured
• 5 years of recently valued company loss runs
• Updated driver information
• Expiring premiums and target premiums for all lines of coverage
